The first step in initiating care is to determine what type of care you might need. The team at SIGMA HomeCare will set up a complementary in-home assessment. We will work with you and your family to determine what daily activities you are having difficulty with, how your medical needs and symptoms are impacting these activities, and what is important to you. Following this assessment, a customized plan will be developed. This plan will outline how much help you need and what types of activities you need help with.
